Forms
We ask that new patients fill out an intake form and background information form so that our Monterey audiologist can best assist you. These simply give us information on your medical history and the issues you are having with hearing so that we can prepare the proper diagnostic tests to identify your issue. They also give you a chance to give us insurance information if you have a policy that covers audiology or hearing aid benefits.
You can save yourself time by printing out the forms and filling them out before your visit, or receive one to fill out when you come to the office.
